Charge Nurse Supervisor / Full Time 40 hrs/per week

ESSENTIAL FUNCTIONS:

  1. In charge of Transition of Care Calls and assuring that patients in are seen for follow ups from hospitalizations in a timely manner for those that are in urgent need of careful follow-up.  Completes medication reconciliation with patient or delegated family member communicating these to the PCP, as well as offering supplementary services as needed. Assists with Care Coordination as needed.
  2. Assists with the Vivitrol program assuring that prior authorizations are completed and patients are scheduled as needed.  Assists with tracking of Vivitrol supply.
  3. Assuring that weekly schedules are maintained, making changes as staff call in and assisting on the floor as needed. 
  4. Approval of nursing ETO, ensuring that staffing is appropriate for needs.  Assists with time sheet oversight to ensure accuracy and avoid OT to excess
  5. Maintain provider Education.  Annual ACLS/BLS/CPR training and renewals. 
  6. Works with WIR updates and correction.
  7. Identifies potential quality improvement initiatives.
  8. Participates in appropriate committees as assigned. 
  9. Assists with error corrections in documentation as needed
  10. Assists with Employee Health, assuring that vaccinations are up to date
  11. Supports all Prairie Clinic policies, procedures and guidelines for care.
  12. Understand and follow all Prairie Clinic employee policies.
  13. Assisting with the Chronic Care program as directed.

QUALIFICATIONS :

  • Registered Nurse (RN), with valid Wisconsin license.
  • Bachelor’s degree in Nursing or Management preferred.
  • Knowledge of physician education requirements and experience in quality improvement preferred. 

SKILLS AND ABILITIES: 

  • Thorough knowledge of the philosophy of principles of general nursing care.
  • Exercises independent judgment and initiative
  • Knowledge of Microsoft Word and Excel
  • Ability to Multi-task.
  • Excellent written and oral and presentation skills.
  • Maintains a pleasant, courteous demeanor when dealing with patients, providers, other employees, the general public and other outside agencies.
  • Maintains patient confidence and protects operations by keeping all information confidential.
  • Abides by all HIPAA regulations.
  • Good interpersonal and organizational skills.
  • Adheres to policies and procedures and serves as model to other staff.
  • Mid level computer and keyboarding skills.
