RN – Triage Nurse – (40/HRS WEEK) 



JOB SUMMARY: Working within clinic policies performs a wide variety of job duties including triaging phone calls to assure patients are guided to appropriate care. Works within clinic policy and closely with Providers to assure that refills and prior authorizations are completed in a timely manner. Able to assist with procedures as needed and to perform a wide variety of procedures independently including phlebotomies, wound care and IV placement and infusions.

 

Responsibilities:


1. Maintains a valid license .

2. Assists patients with refills and completing prior authorizations.

3. Performs a wide variety of procedures including but not limited to phlebotomies, wound care, catheterizations, IV placement and infusions. Explains these procedures completely to patients.

4. Maintains standards of accurate and complete documentation and reporting of pertinent information in a timely manner.

5. Prepares, administers and records procedures/medications and IV solutions skillfully and correctly.

6. Performs tasks assigned in a timely and complete manner.

7. Participates in emergency and triage care.

8. Assists in the development and implementation of new policies and procedures as well as the review and revision of existing policies and procedures.

9. Participates in orientation and in-service training for medical staff personnel including safety education and drills.

10. Are able to assess a patient’s health concerns over the phone and guide them to the appropriate level of care.

11. Must be willing to work a Saturday rotation.

12. Performs INR testing and manages warfarin dosing using practice guidelines as needed.

13. Assists with schedule as needed.

14. Assists on floor at both clinic locations based on the needs of Prairie Clinic.

15. Maintains a cooperative relationship among health care members.

16. Maintains up to date professional and technical knowledge and nursing skills.

17. Supports and enforces all Prairie Clinic policies, procedures and guidelines for care.

18. Attends training sessions/meetings as requested and /or desired for knowledge and expertise.

19. Participates in Prairie Clinic safety education and procedures.

20. Accurately and promptly implements physicians’ orders.

21. Maintains patient confidence and protects operations by keeping all information confidential. Abides by all HIPAA regulations.

22. Understands and follows Prairie Clinic employee policies. 

23. Participates in other assigned duties as designated by the Patient Care Supervisor or Clinic Administrator. 


 

EDUCATION and/or EXPERIENCE:


• Registered Nurse with valid license.

• At least 2 years’ experience

• BLS certified

• Exercises independent judgment and initiative.

• Must be able to multi-task.

• Must be a good communicator with excellent written and oral skills.

• Entry level computer and keyboarding skills.


PHYSICAL DEMANDS:



• Work is performed in a clinic setting.

• This position requires standing, sitting, kneeling, stopping, lifting, bending, walking, carrying, listening, talking, use of a telephone, computer and other basic office equipment.

• Lifting and moving of equipment and patients requires appropriate use of body mechanics.

LPN - RN Full Time (36/hrs week)


Under general supervision, provides direct physical and psychological care and support to patients. Plans, organizes, and performs total nursing care functions within the policies and procedures of Prairie Clinic.

 

ESSENTIAL FUNCTIONS: 

1.    Establishes a compassionate environment for patient care.

2.    Maintains safe and clean working environment.

3.    Provides individualized nursing care to assigned patients based on application of scientific principles, nursing standards and Prairie Clinic policies and procedures.

4.    Prepares patients for physician consult or exam.

a.   Takes medical histories.

b.   Records vital signs.

c.    Explains treatment procedures to patients.

5.    Accurately and promptly implements physicians’ orders.

6.    Maintains standards of accurate and complete documentation and reporting of pertinent information in a timely manner.

7.    Prepares, administers and records medications and IV solutions skillfully and correctly

8.    Assists with or performs treatments/procedures as directed.

9.    Collects and prepares laboratory specimens.

10. Maintains medical supplies inventory and equipment – assuring equipment is in optimal conditions and ready for use. 

11. Makes referral calls and appointments. Notifies patients.

12. Assists as directed with follow up calls and letters, informing patients of lab results.

13. Participates in emergency care.

14. Participates in telephone triage.

15. Assists in the development and implementation of new policies and procedures as well as the review and revision of existing policies and procedures.

16. Supports and enforces all Prairie Clinic policies, procedures, and guidelines for care.

17. Constructively participates in appropriate committees as assigned.

18. Participates in orientation and in-service training for medical staff personnel including safety education and drills.

19. Maintains up to date professional and technical knowledge and nursing skills.

20. Maintains a cooperative relationship among health care teams.

21. Must be willing to work evenings, and weekends as assigned.

22. Attends training sessions as requested and / or desired for knowledge and expertise.

23. Participates in Prairie Clinic safety education, procedures, and drills.

24. Understands and follows all Prairie Clinic employee policies.

25. Participates in other assigned duties as designated by the Patient Care Coordinator or Clinic Manager.

 

 EDUCATION and/or EXPERIENCE:

• Registered nurse (RN) with valid license or Licensed Practical Nurse (LPN) with valid license.

• BLS certified.

mA/cma - Full Time (36/hrs week)

Under the direct supervision of a physician, physician’s assistant, or nurse midwife, provides direct patient care within the policies and procedures of Prairie Clinic.

  6. Assists or performs treatments/procedures as directed.
  7. Collects and prepare laboratory specimens.
  8. Prepares, administers and records medications skillfully and correctly.
  9. Maintains accurate and complete documentation and reporting of all patient information in a timely manner.
  10. Makes referral calls and appointments. Notifies patients.
  11. Maintains up to date professional and technical knowledge and nursing skills.
  12. Maintains a cooperative relationship among health care teams.
  13. Must be willing to work evenings, and weekends as assigned.

EDUCATION and/or EXPERIENCE:

  • Medical Assistant (MA) or
  • Certified Medical Assistant (CMA)

PHYSICIAN ASSISTANT/NURSE PRACTITIONER

DISCRIPTION:

You will be responsible for diagnosing and treating a wide range of medical conditions, prescribing medications, and educating patients about their healthcare needs.


RESPONSIBILITIES:

  • Order and interpret diagnostic tests.
  • Make accurate diagnoses and develop individualized treatment plans.

REQUIREMENTS:



  • Valid Wisconsin Physician Assistant license and or Valid Wisconsin Nurse Practitioner license.
  • Minimum of 2 years of experience as a Physician Assistant and or Nurse Practitioner preferred.
  • Advanced knowledge of medical conditions and treatments.
  • Excellent diagnostic skills.
  • Strong interpersonal and communication skills.
  • Ability to work effectively as part of a healthcare team.
  • Proficiency in electronic medical records and other healthcare software.
  • CPR certification.
  • Ability to prioritize tasks.
